What if my provider is not listed?
If you couldn't find your provider, please fill out this form with the provider's information. We are a local health plan started by local physicians and pharmacists and in many cases, we can reach out to your provider and bring them into the Troy Medicare network very quickly.
If you are getting care from a provider who is not listed, call us, you have the right to request to continue to see that provider if you are under an active treatment plan. We will review your request and make a coverage decision. For more information, see your coverage rights page.
