Interested in selling Troy Medicare’s Advantage Plans?

Here is a list of the required documentation to become appointed with us for Medicare Advantage. Send us an email so we can get you started on the appointment process and we will be happy to provide you with all the required forms and answer any questions you may have.

We will need you to send us copies of the following documents:

● North Carolina Health Insurance License(s)
● Errors and Omissions Declaration Page
● AHIP Training Certificate including FWA Certification

W-9 Form

Once you have completed these steps, and submitted them to us, we will complete your appointment process and provide you all the necessary information so you are “Ready to Sell”.

Enroll your clients through our easy to use online portal:

BROKER LOGIN PORTAL

In our dedicated Broker Portal, you will be able to download a copy of your client’s application at any time and track the application's progress. Don’t forget that you can complete your Scope of Appointment within the Broker Portal!

If you do not yet have access to the portal, please email us with a request and include your contact information including Full Name, Address, Email, phone number and NPN (National producer number).

Phone Enrollments

To complete an enrollment by phone, you and your client can three-way call into our enrollment team at (866) 704-8769. All calls are recorded and one of our representatives will help you and your client complete their enrollment into our plan quickly over the phone. Don’t forget to fax or email your SOA.

Fax Enrollment Applications

Fax us your application here to (704) 235-1689. Please include the application, a Scope of Appointment and any comments/questions you may have.
