Troy Medicare and Isaac Health Partner to Bring Proactive Brain Health Screening to North Carolina Seniors

CHARLOTTE, NC — Troy Medicare and Isaac Health today announced the results of a first-of-its-kind partnership to proactively screen Medicare Advantage members for Alzheimer's disease and related brain health conditions.

Program Results

Since December 2022, the program has used Isaac Health's proprietary detection algorithms to identify Troy Medicare members who may benefit from early brain health intervention — and the results have been striking. Of the members reached through the program, 38% engaged and enrolled. Among those who enrolled, 87% continued participating for at least three months, demonstrating sustained engagement in a population where follow-through is often a challenge.

What the Program Provides

The program provides enrolled members with tailored medical management by specially trained doctors and therapists, caregiver support resources, and proactive care management designed to slow cognitive decline and improve quality of life. Fifty-two Troy Medicare members participated in the initial program phase.

Early detection of cognitive decline is critical because it opens the door to interventions that can meaningfully slow disease progression. Members identified through the screening program gain access to cognitive assessments, treatment planning, and ongoing monitoring — resources that are often difficult to access through traditional Medicare pathways. For many seniors in rural North Carolina, the Troy Medicare–Isaac Health program represents their first meaningful engagement with specialized brain health services. The program also recognizes the profound impact that cognitive decline has on families and caregivers. Expanding the Program

The two organizations plan to expand the program to reach additional Troy Medicare members in 2023 and beyond.
