Troy Medicare Announces 2025 Plan Benefits: $0 Premiums, $0 Copays, and Expanded Coverage Across 34 North Carolina Counties

Plan Updates October 1, 2024

CHARLOTTE, NC — Troy Medicare today announced its 2025 Medicare Advantage plan benefits, continuing its commitment to $0 monthly premiums and $0 copays for primary care, specialist, and urgent care visits across 34 North Carolina counties.

For plan year 2025, Troy Medicare will offer two plans. Complete plan details, including the Summary of Benefits and Evidence of Coverage, are available in the Documents & Forms section.

Troy Medicare (HMO)

Troy Medicare (HMO) features $0 monthly premiums, $0 copays for primary care, specialist, and urgent care visits, integrated Part D prescription drug coverage with a $0 deductible, and a $20 monthly over-the-counter allowance. The plan includes hearing, vision, and dental benefits along with a maximum out-of-pocket limit of $3,950 for in-network services.

Troy Medicare D-SNP (HMO)

Troy Medicare D-SNP (HMO) is designed for beneficiaries who qualify for both Medicare and Medicaid. The D-SNP plan offers up to $2,200 in combined benefits for over-the-counter items, healthy foods, and utility assistance through the Special Supplemental Benefits for the Chronically Ill (SSBCI) program — plus a $115 monthly allowance.

The SSBCI benefits are specifically designed to address the social determinants of health that affect dual-eligible beneficiaries. Many D-SNP members face daily decisions about whether to spend limited resources on food, medications, or utility bills. By providing direct financial support for these essentials, Troy Medicare helps ensure that members can prioritize their health without sacrificing basic needs.

Strategic Focus for 2025

The company's focus for 2025 is clear: expand supplemental benefits, grow the county footprint, and maintain the $0 cost structure that makes Troy Medicare accessible to the seniors who need it most. Learn more about Troy Medicare's mission and values on the About Us page.

Pharmacy-First Differentiation

Troy Medicare's pharmacy-first model continues to differentiate its plans from competitors. Members benefit from integrated medication therapy management, where community pharmacists work closely with Troy Medicare's Care Management team to review prescriptions, identify potential drug interactions, and ensure that members are taking their medications as prescribed. This collaboration is especially valuable for seniors managing multiple chronic conditions, where polypharmacy — the use of five or more medications simultaneously — is common and can lead to adverse events if not carefully monitored.

Enhanced Care Management

For 2025, Troy Medicare is also enhancing its care management outreach programs. The company's dedicated Care Managers will continue conducting proactive health risk assessments, connecting members with preventive screenings, and providing personalized coaching on chronic disease management. These programs have shown measurable results in reducing avoidable emergency room visits and improving member engagement with their own health.

Serving Underserved Communities

The expansion to 34 counties reflects Troy Medicare's strategic commitment to serving communities where healthcare access is most needed. Many of the counties in Troy Medicare's service area are classified as rural or medically underserved, with limited provider options and significant health disparities. Troy Medicare's approach — building deep partnerships with local health systems and community pharmacies — is designed to bring high-quality, affordable care to these populations.

Troy Medicare's 2025 plans are available in Alexander, Anson, Bladen, Buncombe, Burke, Caldwell, Catawba, Chatham, Columbus, Cumberland, Durham, Franklin, Granville, Harnett, Haywood, Henderson, Hoke, Iredell, Jackson, Lee, McDowell, Mitchell, Montgomery, Moore, Orange, Person, Polk, Richmond, Robeson, Sampson, Scotland, Swain, Transylvania, Vance, and Yancey counties.

Medicare Annual Enrollment runs from October 15 through December 7, 2024. Eligible beneficiaries can enroll by calling Troy Medicare at 1-888-494-TROY (8769). For questions, contact Troy Medicare. All plan materials meet CMS compliance standards.
