
Recovering at home is a big part of feeling better. After a hospital stay, it is normal to have questions about what to do next. Here are some simple steps to help you stay on track and recover safely.
Take Your Medicines the Right Way
Your medicines are very important for your healing.
- Check your list: Make sure you have an updated list of all your medicines.
- Ask questions: If you are not sure why you are taking a pill, call your doctor or pharmacist.
- Do not miss a dose: Keep your pills in a place where you will see them, like the kitchen table. Setting a medicine reminder alarm on your phone may help to remember to take them.
Keep Your Follow-Up Appointments
Your doctor needs to see how you are doing after you leave the hospital.
- Write it down: Put your follow-up appointments on a calendar.
- Plan your ride: If you need help getting to the office, ask a friend or call Troy Care Management.
Watch How You Feel
It is important to know when to call your doctor.
- Watch for signs: Look for new pain, fever, or feeling dizzy. If you have an incision, notify your doctor immediately if you have spreading redness, drainage or warmth around the incision.
- Call your doctor: Do not wait if you feel worse. Call your doctor's office right away.
What to Expect After You Get Home
- A Check-in Call: Within 24–48 hours after you leave the hospital, a Troy Care Manager will reach out to you by phone to make sure you have everything you need, like your medicine and medical equipment, and to review your discharge plan.
- Your Medicine Review: Within 72 hours of discharge, your Troy Care Manager or an assigned nurse will work with you to go over all your medicines to make sure you know exactly what to take and when.
Tools to Help You Stay Organized
- Use Your Health Booklet: As a Troy Medicare member, you have a Personal Health Record booklet to write down your health goals, doctor visits, and test results. It is a great way to keep your important information in one place to share with your doctors.
- Nurse Advice Line: Do you have a health question that is not an emergency? You have access to a 24-hour Nurse Advice Line to speak with a nurse about your health concerns.
