Closing the Gap Between Doctor Visits: How Troy Medicare Supports Members Living with Diabetes

Primary care providers today are asked to do more with less time than ever. Appointments are shorter, panels are larger, and it's easy for important details — a missed medication, a side effect, a question a member was too rushed to ask — to fall through the cracks.

That's where Troy Medicare's care management outreach program comes in.

What the Program Does

Our team of pharmacists and nurses proactively reaches out to members living with chronic conditions like diabetes and heart disease. These conversations aren't rushed, and they aren't transactional. We talk through:

  • Medication list reviews — making sure every medication a member is taking is accounted for and working together
  • Potential side effects — so members know what to expect and when something's worth flagging to their provider
  • Lifestyle modifications — practical, everyday changes that support better health outcomes

Filling the Gap Between Appointments

Doctor visits are often too short to cover everything. Our outreach gives members a chance to ask questions without feeling rushed — and we've found that once that door is open, members raise concerns that go well beyond the original reason for the call. A question about a refill turns into a conversation about a new symptom. A medication check-in becomes an opportunity to catch something before it becomes a bigger problem.

Heart Health for Diabetes Patients

When we find a gap in your care, we help you and your doctor fix it.

For example, we have a program to help with heart health. Research shows that people with diabetes often need a specific medicine called a "statin" to protect their heart. Sometimes, this gets missed during a short doctor's visit. We call members and doctors to explain the benefits and see if a statin is right for your care plan.

It's Not Just What You Take — It's How

Many medicines, like insulin for diabetes or inhalers for asthma, only work if you use them the right way. We don't just make sure you have your medicine — we make sure you know how to use it so it does its job.
