Troy Medicare Receives CMS Conditional Approval

CMS Approval of MA, MA-PD Application

The Centers for Medicare & Medicaid Services (CMS) is pleased to inform you that we conditionally approve your Contract Year (CY) 2020 Medicare Advantage/Medicare Advantage – Prescription Drug (MA-Only/MA-PD) application under contract number H4676. CMS determined that you qualify to contract as an MA organization under Parts C and/or D of Title XVIII of the Social Security Act. This conditional approval also applies to any employer/union-only group waiver plans (i.e., “800-series” plans) under this contract number.

In order to enter into a contract with CMS as an MA/MA-PD organization, CMS must also approve your bid, including your formulary, as required by 42 CFR 422 Subpart D and 42 CFR 423 Subpart F. Additionally, you must complete all other required pre-implementation activities, including system and data testing.

This conditional approval is based on the information contained in your application. If there are any changes to the information, or if we determine that any of the information is inaccurate, CMS may withdraw this approval and/or issue a denial notice. Accordingly, you must notify CMS of any changes to the information contained in your application.
